Permission to Use Photograph [Video/Audio]

I grant to Cherry Capital Paddle America Club (CCPAC) and its representatives the right to take photographs [video/audio] of me and my property in connection with CCPAC events, and I authorize CCPAC and its representatives to copyright, use, and publish the same in CCPAC print and/or electronic publications. I agree that CCPAC may use such photographs [video/audio] of me with or without my name and for any lawful purpose, including for example such purposes as publicity, illustration, advertising and Web content. I forever release and discharge CCPAC from any and all claims, actions, and demands arising out of or in connection with the use of said materials, including, without limitation, any and all claims for invasion of privacy, publicity, and libel or slander.
