Application:

We now have three forms to fill out and send to CCPAC (all three links open in new browser windows):

  1. Membership Application Form — Type in your information, print the page, and send it in with your payment. (At the bottom of the Membership Application Form is a link to the ACA Waiver, your next step.
  2. ACA Waiver and Release of Liability (Adobe Acrobat file) — Waiver for Minors — Print it out, fill in your information, sign it, and send it in with your Membership Application.
  3. Media Release Form — This either allows or prevents CCPAC from using your likeness in publications, including CCPAC's web site. You need only fill this out once! If you change your mind after submitting this form, please contact our membership director.

How to Renew Your Membership:

Check your membership date on the mailing label of your newsletter. When it is time to renew, you should see a “Time to Renew” sticker near the label on the newsletter. If you view the newsletter on the web page instead, you will receive a renewal application reminding you to renew. Regardless, at renewal time, please fill out the application form on the back page of the newsletter, sign the waiver on the other side, and return the whole page with your annual dues. We don’t have employees and money to mail out notices, so you need to take responsibility for your own renewal. Don’t delay and don’t miss out on another great season of paddling.

Membership is valid for a one-year period from date when dues are paid.
